The following terms and conditions are agreed between the Client, whose details are set out at the end of this Agreement, and Jake Biggs (Jake), clinical nutritionist, of 25/2A Hollywood Avenue, New South Wales, Bondi Junction, 2022.
1. Scope of practice

Jake offers holistic nutritional consulting and advisory services (the Services) to the Client.

The Services are nutritional and lifestyle advice and recommendations provided with the aim of improving the Client’s health, wellbeing, and vitality.

Jake is not a medical practitioner and does not diagnose, cure, treat or heal any health ailments, diseases, or illnesses.
2. Services

Jake will provide the services in good faith and based on his expertise and experience.

The Client acknowledges that unless specifically disclosed to Jake by the Client, Jake will be unaware of any individual allergies or other reactions that the Client may have to any nutritional supplement or food recommended by Jake.

If the Client has any pre-existing medical conditions, they should discuss them with their medical practitioner.

The Client will make their own decision as to whether to follow any of Jake’s recommendations and should the Client choose to do so, they acknowledge that it is their personal choice.
3. Indemnity

It is a condition of Jake providing the Services to the Client that the Client indemnifies and holds Jake harmless from and against any cost, expense, illness, loss or liability of any sort that the Client may suffer or experience as a result or in connection with the Client following any of the advice and recommendations provided by Jake in the form of the Services.
4. Privacy Statement

Any information or data provided by the Client is protected by the National Privacy Principles under the Privacy Act 1998 (Cth) as amended by the Privacy Amendment (Private Sector) Act 2000 (Cth).

Jake will not disclose any private details or information provided by the Client with anyone unless first obtaining the Client’s permission.
